Field Trip Medication Form

Directions:

1. Complete one form for each prescribed medication.
2. Forms must be completed by a physician or nurse practitioner.
3. All medications must be in their original prescription or over-the-counter container.
4. Over-the-counter medications must be in a new, unopened container.
